About: South Eastern Health & Social Care Trust / Condition Management Programme

(as the patient),

I suffer with chronic illnesses. I was referred to Community Management Plan as those illnesses were affecting my work life and I was in danger of having to leave work. 

I followed the steps laid out for me by my clinician and am on a strong road to recovery.

Thank you very much for sharing your story of the Condition Management Programme which has been shared with the team and will hopefully resonate with others in a similar position and encourage them to access support. I am pleased that you were referred and had access to CMP the health, work and wellbeing programme when your illnesses were affecting your work and you were in danger of leaving work. I am delighted that the support provided has helped and that you progressing in your recovery.
