hi Breidge,

I am Celina McConnellogue ward manager of ward 23.

Thank you for your kind message, you have been through a very difficult period with multiple fracture. It will take time to get back up on your feet with these but I'm sure you will do well. I wish you a speedy recovery

Dear Briedge,

My name is Ann Marie Conlon and I am the Falls Integrated Pathway Co-ordinator within the Western Health and Social Care Trust.

I am very sorry to read that you had a fall at home and that you sustained very serious injuries that have caused you to feel depressed at this time as well as being sore.

Falls can happen for many reasons and it is important that the contributing factors for having a fall are identified. It is also important to address the cause of your fall to reduce the risk of you having a further fall and injury.

I hope that your pain is managed well at this time and that you are receiving rehabilitation to support with your recovery.

It was reassuring to learn that your aftercare and carer were good.

As an organisation we are committed to consistently show kindness, compassion, professionalism and respect to the patients we care for.

I hope that as you recover that your feelings of depression improve.

There are a range of resources available on the Trust website related to falls and supporting services which can be accessed through the link https://westerntrust.hscni.net/health-and-wellbeing/later-years-2.

As this feedback system is anonymous, if you would like to talk in more detail about your fall, the impact and your experience of services received or the services available to provide support, please e-mail me, annmarie.conlon@westerntrust.hscni.net or alternatively contact the Falls Integrated Pathway office on 02871610763, leaving your name and number and a member of staff will get back to you as soon as possible.

Thank you for taking the time to share your feedback on the service received through Care Opinion. We are always grateful to those who share their experiences of the services they have received.

We are currently looking for interested members of the public to provide feedback on our Falls Prevention Resources. This can be someone who has had a fall, is at risk of a fall or someone who cares for a person who has had a fall and feels passionate about improving Falls Services and Resources in the Western Trust. If this is something that you would be interested in and would like more information on what is involved, please contact Una McGovern at involve@westerntrust.hscni.net or call 07775993582.

Best wishes,

Ann Marie Conlon

Falls Integrated Pathway Co-ordinator
