"A great service"

About: Older People’s Services / Hospital at Home Northern Sector

(as a staff member posting for a carer/relative),

It is a great service. My mum has dementia and it was amazing that her care was able to be done at home instead of being hospitalised.

Prevention of admission to hospital is the aim of our team and even more so when someone has dementia.

Patients with dementia get more confused and upset if taken out of their own environment which can lead to many other risks and issues. Nursing your loved one at home has given you the opportunity to spend more quality time with them.

I will pass on your feedback to all the staff.

Thank you for sharing the positive experience that your Mum and your had of the journey through the Hospital at Home Service.

The Hospital at Home Service is a vital service particularly for people living with dementia, to ensure continuity of care is maintained in their established care environment, without any distressing transfers to hospital.

Person centred care is paramount within dementia care and this has been evidenced through your experience.

Your feedback is very welcome to ensure safe, effective, compassionate and well led care is delivered.
