(as a staff member posting for a carer/relative),

Hospital at Home was quickly involved with mums care. the Dr gave us detailed information and was easily understood.  The nursing team provided excellent care for mum in the comfort of her own home.  Individualised care and we were able to spend time ensuring she was comfortable.  We feel that the team were focused on mum and not having to worry about patients.  It was also convenient for family to visit-less stressful than back and forth to hospital.  Excellent service.

Thank you for your feedback.

Nursing patients in their own home is the aim of the team and I agree it proves much more beneficial for both family and patients. The team will at all times involve family and patient in the current plan of care.

The team are able to focus solely on one patient during each visit without distraction.

I will pass on your feedback to all the staff.
