On the face of it the hospital seems like it is a good place that is their to help. After a couple of days it can feel like a comfortable and safe environment. If you don't need any extra care after leaving hospital then it will be fine. However if you do require assistance after your discharge, this is where you will be let down. As soon as you are discharged you are on your own. You a discharged, without any of the correct paperwork. You have no way of getting any help. They kick you out. The hospital will leave you feeling abandoned with no idea of where to go or what to do.

Matt,

I was very sorry to read about your experience. Clearly, this should not happen as you should not be discharged without a care plan which details what should happen when you are discharged, the support you will get and where to get help in the event of a crisis or relapse. Someone else (for example a friend or member of your family) should be involved in your discharge with your consent so that they can also support you. All discharged patients should get a follow up call from our Crisis Response and Home Treatment Team within 48 hours to check how you are getting on.

It sounds as if this has not happened in this case which is regrettable, for which I can only apologise.

You have a number of options open to you and our Patient Advice and Liaison Service Manager will be happy to discuss these with you, whether you need additional support or would like him to look into your case in detail so that it does not happen again and we can give you a satisfactory explanation.
