As it is Stroke Awareness Month I want to share our family experience of my father’s Stroke Journey from ED to his move into the community.
On arrival to ED he was met by the Team and quickly assessed by the Stroke Lysis Team and Dr Tallon. The care and treatment in partnership with the MDT within CAH Stroke Ward was professional and caring. There were clear lines of communication and always someone willing to listen.
A speedy transfer for ongoing Rehab to Lurgan Hospital was arranged and Dad received a detailed and comprehensive assessment by the MDT rehab goals set and his wishes and views listened to. As a family we were actively encouraged to participate in rehab and support Dad through a frightening time this also helped us as a family.
Discharge was discussed and planned further impute was given by a professional and supportive Community Stroke Team. Dad’s progress was evident. District Nursing and Trust Home have been providing on going care and it is obvious that that the joint approach has helped us all. I wanted to share our thanks for all the help my father and us as a family have received.
