"The staff were so caring and patient"

About: Intermediate and Transitional Care / Community Rehabilitation Team

(as a service user),

I fell and broke my hip. Initially when I was discharged from hospital I really struggled and it wasn't until the rehabilitation team got involved that I slowly seen a light at the end off the tunnel . The service they provided was fantastic and the staff were so caring, patient and allowed me to go at my own pace.

My first contact was with Rachel the occupational therapist. She was amazing putting me at ease and explaining the services they provided. Rachel assessed my individual needs and supplied a toilet frame, raised toilet seat and a perching stool, my first steps to independence . She also explained that a physiotherapist would be in touch and that the rehabilitation team would be calling every day. The care team i couldn't thank enough helping me with personal care and also assisting with meals etc.

Then Paula the physiotherapist called, assessed my mobility and put a plan tailored to my individual needs in place. This included not only supplying a zimmer frame but also making sure i was confident and safe using it. Paula put an exercise plan in place, adding this to my care plan. From there on in the rehabilitation team helped every day with these exercises. As I got stronger, Paula added a few more. Once my mobility improved, Rachel the occupational therapist called, helping me with showering and making sure I was confident and safe.

In conclusion i moved from the zimmer to a crutch and am now using a stick,  next step is freedom. I cannot express enough how grateful and thankful I am to all the team. No matter how trivial something may seem i can't stress enough every thing from mobility aids to exercises recommended, please take it on board. Thank you to all the team, i'd have been lost without you all.

Thank you for taking the time to provide such a detailed overview of what our service entails and provided for you.

As one of the managers of the service, it is lovely to hear how Rachel and Paula helped you in your rehabilitation journey alongside the rest of the team. I am delighted to hear that you really benefitted from the individualised approach that the team takes.

I will ensure to share your feedback at our staff meeting this week as it is heartening for all staff to hear about the impact they have on their service users.
