About: Older People’s Services / Hospital at Home Northern Sector

(as a service user),

Last year I had the pleasure of being put under the care of this amazing team. Just a few weeks ago after a very nasty fall I had them again look after me.

From the consultant, the nurses, the O.T, the fact that they bring the hospital care to you in your home, instead of going into hospital, is amazing.

Their care is exemplary and I would like to see more teams in the scheme.

My family can't thank them enough for the care they gave to me.

My name is Ann Marie Conlon and I am the Falls Integrated Pathway Co-ordinator within the Western Health and Social Care Trust.

Thank you for taking the time to share your feedback on the Hospital at Home service. We are always grateful to those who share their experiences of the services they have received as we can use your feedback to identify areas of good practice as well as areas for improvement.

It was reassuring to learn that you received an excellent service at the time of attendance.

As an organisation we are committed to consistently show kindness, compassion, professionalism and respect to the patients we care for.
