"Medication issues and a near miss on morphine"

My story starts after joining the practice last year. I suffer with chronic pain, yet trying to get anyone to help with my medications has been impossible.

I have telephoned, been into the surgery and yet still nobody seems to be able to get my medications right. I have on numerous occasions requested a pharmacist to call me to sort my medications out, yet it's too difficult.

Medications have been changed, without so much of a phone call, and more recently,  following an admission to hospital, 4 telephone calls and 2 visits to the surgery - another big mess up with medications.

I needed an increase in a particular medication, but not done. To top it off, if I'd have taken the dose of medication prescribed rather than the dose recommended, I could've died, I'm a retired nurse due to ill health,  it's a good job I knew not to take that dose.

And complaining to the surgery, nobody seems to care. When I phoned I was told I would get a call back ASAP, but seemingly not important enough on a near miss with morphine. 

I want to move practices, but in the middle of referrals. If I gave this kind of care as a nurse, I would have been utterly disgusted with myself.
