"Poor discharge but lovely community team"

About: Integrated Community Services Service Delivery Unit / Pershore and Upton Neighbourhood Team Worcestershire Royal Hospital / Trauma and orthopaedics

(as a relative),

The discharge from hospital was dreadful with my husband was discharged at 4:00 PM. He's over 90 and I'm over 80.

We were told that we would receive some equipment and physiotherapy occupational therapy within the two hours. Since being at home we had nothing for the next two days that's 48 hour's before any equipment was delivered to our home.

After about 27 phone calls to my GP the PDU unit at Worcester hospital & the ward I finally managed to get through to somebody called Merrick who was really helpful and an OT arrived.

They arrived with a toilet frame however the OT seemed quite defensive because the toilet frame didn't fit and they said they only work on certain days, and only part time.

Since then we've had amazing care from the community therapy team they've been absolutely fantastic their help has been amazing and they could not have done more (Pershore neighbourhood team and one from Upton)

Sheila the physio all the nurses included Yvonne and Joan have been amazing there was a slight hiccup with a bank nurse one day but apart from that they've been absolutely fantastic and I want to say thank you it's just a shame that there port discharge from the hospital let down this episode of care.

Good morning,

I too was sorry to read of your experience around the discharge of your husband from hospital and see that you have contact numbers /email to discuss further.

It is rewarding to hear that as a neighbourhood team we have been able to support your husband at home, bar an occasion you describe as a "hiccup" - if you have any concerns with this, please do ring the team and ask to speak with me.
