"Worst practice ever"

My 89 year old mum has had no care while at this practice. No one ever answers the phone whatever day or time. Promised referral to memory clinic 3 times and it was never followed through and I was unable to find anything out as I live in Leeds and rely on the phone which they don't answer. Appointments impossible to get and delays and problems eith medication requests. I have moved her to another practice now and the difference is unbelievable.
