"Denied ADHD referral"

About: Community Mental Health Services / Mental Health Acute Assessment and Treatment Service (MHAATS)

GP requested a referral for ADHD assessment and I was turned down. Psychiatrist reported that I don't have ADHD but instead something else based on extensive, and documented, childhood presentation, meeting all symptoms, scoring highly on all clinically recommended questionnaires, 3 attempts by a GP to refer, and nothing else, not even meeting me. Complaint via the NHS led to being told that actually Forth Valley have no ADHD pathway for adults.

In the initial correspondence between the GP and the mental health team, the GP was informed that the mental health team also needed to see evidence of childhood presentation, which is completely inaccurate given that no pathway exists anyway even if this is present. Therefore, it seems that even the staff working for the health board have no idea that there is no adult ADHD pathway available.

This has now been taken higher, given what I feel is the significant public impact. Coming to this conclusion based on these points only has a significant impact on a patient who has reached a point of seeking out a diagnosis. Indeed, having this response from a psychiatrist when it has not been based on a full review of the patient's records and has no discernible basis will in many cases prevent a patient from ever seeking a diagnosis again, which given the significantly increased risk of suicide, depression, and substance misuse has the potential to have a devastating and continuing effect on the patient. 

Awaiting response from the ombudsman before initiating legal proceedings for what I believe to be negligence.

I’m sorry to read about your experience. Unfortunately the information you have been given is correct and our services do not have a pathway for ADHD assessments. Secondary care community mental health teams receive referrals for a wide range reasons and in keeping with other specialties in secondary care, do not accept all referrals received.

We are working on an ADHD pathway and have a wide range of staff engaged in that process, including colleagues from general practice.

We hope that this, when complete, will help direct people to the right support and avoid the frustration and disappointment you have experienced.

This seems to be missing the point entirely. I am well aware that the pathway doesn’t exist (although not in anyway due to information provided by the NHS) - I have spent 8 months providing evidence to the ombudsman and a legal team. The issue is not the lack of ADHD pathway. Obviously private assessment is possible. The issue is being told outright by a psychiatrist that I did not have ADHD but something else, based only on a GPs request for an ADHD referral and 92/100 score on the 3 ADHD questionnaires and attempting to diagnose someone based on this, which is not only unprofessional but goes completely against good medical practice.

I can only assume that the fact this is the status quo means that there is a collective belief of decision makers that all the people receiving this same response aren’t going to discuss it on social media forums and take collective action once they realise they are being led to believe that they don’t have a medical condition…which is unwise

For what purpose? I’ve already been through PALs and the conclusion is due to be reached by the ombudsman at the start of May, after which I’ll be bringing legal action.

I’ve already given ample opportunity for this to be addressed at all levels of the NHS and it has not been.
