Mum was pulled over by her dog and broke her hip during the fall
She also has a diagnosis of alzheimers dementia.
Due to a combination of physical frailty , cognitive impairment and lack of motivation she is unable to undertake any daily living activities without promoting and assistance.
She does not eat or drink enough
She has medicine s for a range of other physical health issues too.
We are concerned for both her physical and mental health.
She is in her own home with support from carers cleaner, meals on wheels, family etc but it is a fragile situation
Her GP is excellent and really understands mum's issues and works close with us and other professionals to get the best outcomes for mun
We have received fantastic support from the frailty team and from the ict therapy team to adcress both short term acute needs as well as long-term needs.
Mum had an impressive and thorough OT assessment recently, which was helpful to both mum and the family. Mum's voice was heard as well as input from me and other professionals. Mum was treated with dignity and respect.
A clear plan which will help mum to live as independently as possible was agreed
