"Persistent prescription mix-up"

(as a service user),

I have recently had my ADHD medication changed and as you can imagine it is imperative that they are taken as when they are prescribed. 

It is like whoever does prescriptions goes out of their way to either not have it ready or mess it up or say they know nothing about it, when there is obviously an underlying feeling between reception staff and doctors the only people who suffer are patients .

I am lucky enough to be able to phone my NHS psychiatrist and they phone the practice, which is absolutely ludicrous. This has been going on for years.
