Yvonne frailty practitioner and Ellie paramedic attended my fathers house after he had fallen out of bed and couldn't get up. They spent almost 3hours with him fully examining him and liaising with the hospital team as to best next steps throughout. They ensured dad was included in the discussions we cannot thank them enough
