"Gave my mum some independence back"

(as a carer),

My mum was recently in Monklands Hospital. I was really worried about her coming home.

Jennifer from the home assessment team and explained that my mum could have carers at home. This was such a relief as my mum had been struggling for a while.

Jennifer explained everything to me and even came and saw my mum the next day when she was home. Jennifer got her some equipment to make things easier for her at home.

The care and communication was amazing and I'm so grateful to Jennifer and the carers that gave my mum some independence back.

Thank you for taking the time to share your experience and gratitude to members of our wonderful Home Assessment Team (HAT). Starting to think about discharge following a period in hospital is daunting, especially when you say your mum had been struggling for a while at home.

It is really great to hear that Jennifer was able to reduce your worry about your mum's discharge through her communication as well as her practical support to make things easier at home and to support mum's independence.

We are very lucky in HAT to have Jennifer with all her skills and experience. Our carers are a credit to our team and we couldn't do what we do without this amazing team.

I have passed on your comments to Jennifer as well as the wider team to remind them of how valued they are.

I wish you and your mum all the very best in your journey ahead.
