My elderly and frail father, with both acute and chronic health conditions, was recently discharged from NHS Forth Valley hospital. Communication with the family about his conditions, treatment and care needs had been inconsistent. We had severe concerns about his discharge from hospital and his ability to cope at home. He was still in great pain and quite immobile.
He was extremely fortunate the he was given Hospital at Home support for more than 2 weeks after discharge from the ward. This was a great example of a multidisciplinary team working together with patient’s needs at the centre of their approach. Each team member was interested, showed care and compassion, and worked hard to solve problems with mobility and medication. They were always professional, did what they said they would do and often more. From the family’s point of view, the Hospital at Home team communicated clearly and regularly. We were encouraged to get in touch with any concerns, and reassured about progress.
Dad is still frail and unwell, but managing at home with appropriate support. This would not have been the case without the excellent service provided by NHS Forth Valley Hospital at Home Serice.
