"I am worried I have maybe been forgotten about"

I last received treatment early this year and was informed I would hear back from urology soon after. I did receive a call, approximately a few months after my treatment, from a urology ANP asking me how things were going and that they would put me on the waiting list to be seen by the consultant I am under - I was explained this was a bit of a wait. I have tried to find a contact number/email to investigate when my appointment could be but I have been unsuccessful.

As I have not had any further confirmation that I am on a waiting list, I am worried I have maybe been forgotten about. I understand the pressure the NHS is currently facing with waiting times. I am sorry if this is bothersome, but would greatly appreciate any information regarding this matter as I feel my condition is worsening and attending my GP would just be a re-referral to urology.
