I was referred to the Community Heart Failure Specialist Nurse following a echocardiogram that indicated my heart function was 30%. Although the hospital cardiology nurses were supportive, they were very busy and I felt that I was just one of many patients going through the system.
When I was referred to the Community nurses that all changed. everything was explained to me really carefully and all my questions and concerns were answered. The introduction of my medication and the support around side effects has been excellent. Over the past year the management (joint between myself, community team, GP and hospital) has enabled my health to improve dramatically. I feel that I am in control of my daily health needs (and lifestyle changes) but always have the support of the community nurse. I can't praise the community service highly enough - particularly the two nurses who have been so key to my life improvement - Joanna and Louise
One thing I would want to change is the job title of the specialist nurse. I understand from a provider perspective why you chose "heart failure" but from a patients pair of view it sounds negative and time limiting (life)
