(as a service user),

The DN team supported my grandparents who were very frail and elderly and helped us to keep them at home by providing wound care as needed, support and advice and helped us to get equipment as needed to help support them to remain at home.   The DNs then helped the myself care for my grandmother within her last few hours of life and gave great comfort and support and I will forever be grateful thank you.  A precious service not to be forgotten as I feel they are the unsung hero’s of the NHS!

I cover the north locality, working with the community nursing teams to ensure the safe and effective delivery of patient centred nursing care to patients in the home or homely setting. My role includes direct patient care; assessing and monitoring the health needs of patients and their families, planning and coordinating care in collaboration with patient, carers and the multi-disciplinary team.
