"In patient discharge procedure not being followed."

My daughter was discharged very suddenly. The inpatient discharge policy was not followed. The family were not involved. We have not been given sight of the discharge plan or advised of any risk assessment or crisis actions.

I have found the planning and hand over to the community team has been rushed, is shambolic and virtually non existent.

This could massively impact on my daughters life once again. Good careful planning would have given her the best chance to stop her being re admitted again and again. It would also save the trust thousands of pounds in care costs for these re admissions by better hand overs.

The procedure was written to make sure these things happen.
