My grandmother who has dementia had stopped eating and drinking due to oral thrush. The Acute Care at Home team promptly came out on a Sunday to assess her and which made us feel relief as they took control of the situation. They were able to put up subcut fluids until my grandmother could eat and drink again. This gave us reassurance she was being hydrated when she was so unwell.
Very professional staff (both nurses and doctors) who visited 1-2 times each day whilst she was under their care. Great care and communication provided - especially Dr Nicholson who even phoned us late in the evening to update us on her care.
Such a great service which allowed my grandmother to be cared for in her own home without a hospital admission and thankfully back to her own self, which would definitely not have been the case if the acute care at home team didn’t intervene.
