"providing a stool specimen for testing"

(as a parent/guardian),

the GP advised my son to take a stool test, the first specimen was given in only to find out after 2 months that it has been lost on the way to the lab. the second specimen has been given in only to find out after 3 weeks that the label with patient details was missing, (knowing that it was in the bag when handing over), wondering how the front desk took it without checking that all is correct. it is now 3 months that we are waiting for a specimen test but with no luck and a incompetent system.

I am very sorry to hear that your son's specimen was lost and that it took two months before you were made aware of it.

I am not able to find out where the details on the second specimen went missing, our reception team usually check that the patient details are with the specimen and I'm sorry to hear that this was not the case. I understand that this must have been very frustrating for you.

I will discuss your concerns with the staff and reiterate to them the importance of checking that the patient's details are enclosed when samples are handed in.

I hope that your son has managed to get the tests he needed.
