"Treatment of my mother"

About: District Nursing / Dundee District Nursing General practices in Tayside Occupational Therapy Tayside Community Services / Community Macmillan Service

(as a relative),

My mother was misdiagnosed by the practice Nurse Practitioner as having a chest infection. In fact my mother had lung cancer which had spread to her liver, and lymph nodes and a lesion on her kidney.

The COPD nurse failed to follow up on their promise to refer my mum to the district nurse services, leading to untreated bed sores. Occupational Therapy didn't come and see my mum until she was bedridden. She was then given a mattress to prevent sores, a toilet frame, step, bed rail, shower pole, commode, shower grab rails and a wheelchair. She was unable by then to have a shower.

The COPD nurse told the MacMillan nurse that they came to see my mum regularly. They had been once. When I called the district nurse out, they turned up eight hours later via the Hub, stating to me that they had no record of my repeated calls or voice messages that I had left.

District nurses also said to me that district nurse doesn’t do stick-ons when I called to say my mum’s pad had come off her lower back.

Alana716,

With apologies for the delay in this response. I have passed your feedback to the manager responsible for the community COPD service in Dundee. She is currently on annual leave, but is very keen to speak with you further about your experience with the service so that she can investigate further. To allow her to address the concerns raised she would be grateful if you could contact her directly abigail.andrews@nhs.scot or via NHS Tayside feedback at tay.feedback@nhs.scot

with kind regards

Allison

Dear Alana716

I shared this Care Opinion Story with the Dundee Community COPD Team at a meeting last week. Your mothers COPD Nurse recognised this story, and I have been able to investigate this further. I would welcome the opportunity to discuss this with you if you could forward me a contact telephone number or email address. My contact is Tay.dundeecopdservice@nhs.scot

I would like to reassure you that we take all feedback seriously and let you know that I have used the information gained to meet and discuss this incident with nurse involved. Together we have reflected on this event and identified learning for the nurse involved and for the team as a whole.

With kind regards

Abigail Andrews

Dundee Community COPD Team Leader

(On behalf of Abigail Andrews - Dundee Community COPD Team Leader)

Alana716,

I understand that Gail has been in touch with you directly to update you on the changes which have been implemented as a result of your feedback. Communication processes have been reviewed, links with our District nursing colleagues have been improved in relation to pressure area care and we have begun discussion to improve our palliative pathway for patients.

Thank you for your feedback which we hope will lead to improved care for all of our patients.
