"Person centred care"

About: East Kilbride Locality Support Service / Care at Home East Kilbride Locality Support Service / ICST (Community Nursing, Physiotherapy, Occupational Therapy) General practices in Lanarkshire University Hospital Hairmyres / Emergency Department University Hospital Hairmyres / General Medicine

(as a relative),

My mother has been unwell with terminal heart failure for some months and her condition was palliative these past few weeks. The staff in every part of the care system demonstrated realistic medicine and person centredness in all they did for my mum.

After her last Hairmyres hospital admission, where she was so well cared for in A+E and Ward 10, she decided that what mattered to her was staying at home and being with family. That was enabled by the GPs (Calderlea practice), District Nurses, Carers, the home oxygen service and the Palliative care team (led by Vicki who was the most helpful compassionate coordinator of what was needed).

We agreed that we would go to a Hospice when staying at home was no longer possible and the staff there only knew my mum for a few days however enabled what can only be described as a good death in a peaceful environment. Thank you to everyone for all they did.

Dear lanarkshire carer daughter

Please accept my sincere condolences following the death of your mother.

It is humbling to read your comments especially as you have taken the time to write and feedback after a sad and difficult time. Thank you.

I am pleased to read that your mum was well cared for and that her her wishes were the priority. It seems that various members of the multidisciplinary team have supported her care in a person centred approach.

I will share your comments with the wider team as well as those staff mentioned.

Thank you again for your kind words.

Kind regards

Lise
