"It truly is an invaluable service"

About: Older People’s Services / Hospital at Home Northern Sector

(as a carer),

My elderly mum became unwell recently. Local GP referred to H@H team. Within a few hrs she was attended by a Dr and a nurse who carried out a comprehensive assessment including observations, ECG and blood tests.

This important service ensures that patients , where possible, can be nursed at home without the need for hospital admission. 

The team checked in on a regular basis throughout the next few days, providing much needed support and reassurance. It truly is an invaluable service and we are extremely grateful to the wonderful team @ Western Trust Hospital at Home

The aim of the H@H team is to prevent admission for the older person and treat at home as much as possible. I am delighted to hear we were able to prevent your elderly mother from admission to hospital.

The GPs work well with the ream and refer on a regular basic, again preventing admission. All patients receive a comprehensive assessment on admission and are assessed by DR and nurse and a full plan of care put in place.

We also have a OT and pharmacist within the team that link with patients as required. We provide regular visits for patients and staff give regular updates for patient and family.

I will pass on your positive feedback to all the team.
