"Trying to access services"

About: Forth Valley Royal Hospital / Accident & emergency General practices in Forth Valley NHS 24 / NHS 24 (111 service)

(as the patient),

I fell in the garden around broke my glasses and grazed/ cut my face.  I take steroid inhalers so have  skin which doesn't heal quickly.  2 hours later still bleeding I phoned NHS 4 who sent me to A&E. Arrived late in the evening to be told 6 hour wait and the triage nurse would see me shortly. I saw nurse at midnight, took BP and temp and said they would see me again soon.  They never did.

About four hours later I asked reception for update, they predicted a 2 - 3 hour further wait.  I had stopped bleeding,  was exhausted, hadn't eaten since lunch time.  The snack and drink machine was broken, so  I decided to go home get some food and sleep and try after I'd slept.

Next morning I couldn't get a GP or nurse appointment for a week,  so I phoned NHS24, eventually spoke to a trained nurse practitioner who decided I didn't need to be seen.   I said my facial injuries meant I could not take my essential steroid inhalers which my consultant told me not to stop for any reason and always to carry spare.  This nurses opinion was that it didn't matter if I didn't take steroid inhaler for several days or a week.

I contacted my GP surgery again who suggested I try treatment rooms at other local surgeries.  None could see me for a week.  I did manage to speak to a District Nurse who asked me to email photos of my injuries.  They advised me to phone again if it got worse , started bleeding or didn't heal. They also gave me advice on reducing swelling and taking my steroid inhalers 3 times a day as prescribed.   I later discovered from other nhs staff that because a bike race was on local NHS staff had been sent to the route in case there was an accident.   So I, a local resident, couldn't receive medical treatment because some important visitors might be injured!!!   Where do NHS forth valley priorities lie?

I felt ignored, abandoned  and uncared for.  I felt the NHS treatment of me was anything but caring.

I felt the nurse at NHS 24 had no understanding of the importance of taking medication, i.e. steroid inhalers, even though I told them of the instructions not to stop taking this medication.   I also felt they had no understanding of the fact that constant steroid use meant I was immunocompromised with regards to my skin, bleeding and difficulty healing,  even though I explained this .

Dear sigmafp99

Thank you for sharing your experience on Care Opinion. I'm sorry to learn of your fall and the injuries you sustained. It sounds as though you have had quite a worrying time and I really hope you are doing better. I'm disheartened to hear you felt ignored, abandoned and uncared for at this time. This is not the sort of feedback Health Boards like to see at all.

I read your story with concern and would like the opportunity to review your contact with NHS 24. If you would be kind enough to contact me directly with some additional detail, then I could arrange this. I would then share our findings with you. This would allow us an opportunity to establish if there was anything we should have done differently or any learning for our staff.

You can call me on 07814 299944 or email Patient.Experience@nhs24.scot.nhs.uk

I sincerely hope to hear from you further.

Take care meantime.

Shona

Dear sigmafp89,

Thank you for taking the time to share your experience of the Emergency Department and I’m sorry to hear that at the time you attended the department there was an extended wait for assessment during what must have been a worrying time for you. Whilst I can only comment on the Emergency Department aspect of your experience, I am sorry that you had such difficulty accessing healthcare services.

I would like to sincerely apologise for the length of time you waited to be seen and the way you felt through this episode of care, it is by no means the level of care we want to provide for our patients. We are currently experiencing significant demand on our services and we are continuing to work tirelessly to prevent long waits and experiences such as the one you have described.

It might also help to explain that we see patients in clinical priority and not time order, following triage, patients are assigned a triage category from which we have to prioritise the sickest patients first and this may have had an impact on the length of time it took to see a Clinician.

If you would like to speak to me further regarding your experience, please email me on ric.atalla@nhs.scot.

Once again thank you for sharing your experience and I do hope you are recovering well.

Kind regards

Ric Atalla, Operational Manager, Emergency Department, Forth Valley Royal Hospital

Hi sigmafp89

Thanks so much for getting in touch with me directly and sharing more of your experience. I hope you are doing so much better. It was good to hear from you. As explained, I requested a review of your calls to our service which I understand has now been completed.

Thank you also for speaking with a senior clinical staff member who explained to you the findings our our review. I am pleased to note this has found that your calls to NHS 24 were managed appropriately with no learning identified for our staff. I hope you found this reassuring.

With best wishes.

Regards

Shona
