"Concern about Children's Mental Health Service"

About: Children's Specialist Health Services / Child & Adolescent Mental Health Services (CAMHS)

(as a parent/guardian),

 I wish to highlight strong concerns regarding aspects of the referral, assessment and information gathering processes. My son’s well-being and quality of life have been severely impacted due to a significant deterioration in his mental health during waiting periods. I am writing to you in advance of any formal diagnosis being made as I feel there may be missed information presented in his current case. I have been unable to access any support and believe there to be missed information presented in his current case.  

I would like to request that someone in charge and not actively involved review details of his care and support.
