"Lack of clarity in discharge notes"

About: Queen Elizabeth University Hospital Glasgow / Accident & Emergency Queen Elizabeth University Hospital Glasgow / Acute Receiving Unit (Units 1-5) Queen Elizabeth University Hospital Glasgow / Blood vessel problems ( Vascular Ward 11a &11d)

(as a service user),

I was taken to A&E with concerning symptoms such as symptoms of confusion, dizziness, stuttering and nausea.

A&E were great; they said I would be checked to rule out a TIA or inner ear infection.

I was then moved to ARU 1 (same day), where the Drs said the same and again, when moved to ARU 3 (also same day). Everyone within these 2 units were also fantastic.

I was moved to level 11D under the care of the boarders team. The nursing staff were kind, caring and considerate throughout. The MRI was performed yesterday afternoon; I asked the radiographer of this will pick up a TIA, to which they replied yes.

The discharge letter issued yesterday evening makes no note of whether I did or did not have a TIA.

I was hoping the query regarding the TIA would have been clarified within the discharge letter. I found out that I am legally obligated to inform both DVLA and my car insurance company of a TIA. If I did have a TIA, I cannot drive for a month. 

I am grateful for the care and attention received as it turns out my cholesterol is high. I was on blood thinners during my stay.

The discharge letter says the MRI showed I did not have a stroke - this was evident as I had no weakness of limbs, drooping of one side of face or loss of vision. I tried calling the ward but the phone rang out (it's a busy ward so this is to be expected). I would like my query regarding TIA be answered.

Dear Manager Nicole,

Thank you for your reply. I cannot get into my emails at the moment due to a technical hitch but hope this to be resolved tomorrow.

I'm limiting my driving as I still have good/bad days re slight confusion and balance issues. My balance medication finished yesterday.

I really would appreciate clarification on the points I raised. I'll send my details to the email address PatientExperience@ggc.scot.nhs.uk once I get back into emails.

Dear Senior Charge Nurse Karen,

Thank you for your reply. That time is a bit hazy so I cannot remember every name but hope you can pass my thanks to Dr Keir (A+E), Nurse Bernie (ARU 3), Nurse Olivia (11D) and a special thanks to Student Nurse M.Hayder (ARU3). M.Hayder was very kind and professional - I'm sure he'll go far. Apologies if I have misspelt names. All the nursing staff and Drs within A+E, ARU1 and ARU3 were great. The wards were kept immaculate by hard working staff members so there were no issues (as it has been publicised in the past) on that regard.

It's 2 weeks since I was discharged but I still haven't been given the information I require. In my opinion, they're putting patient safety at risk.
