(as a service user),

I would like to express my gratitude. And thanks for the care and support I have received for many years from Dr Richard 

The care from Dr Afuakwah (GP), Alison and Carrie the practice nurses has always been exceptional, supporting me in my changing health needs over the past 5 years. 

The care and support I have received has been caring, supportive, understanding with diligence and respect from everyone including the receptionists Sheila and Rosemary who are always happy to help. 

I would like to wish Dr Afuakwah and Alison, practice nurse all the best for their retirement. 

Additionally, I just wish to thank them all as I will be moving practice due to moving out of the practice area.

Thank you for taking the time to leave us your feedback on Care Opinion.

It was lovely to hear of the support and guidance you have received from the staff within your GP Practice over the last five years with your changing health needs.

I was glad to hear the staff have been professional, caring and most importantly respectful throughout your visits and have gained your trust through time and effort.

I will be sure to pass on your thanks and well wishes to the staff who I'm sure will be touched to read your kind words.

I hope you continue to receive the fantastic support your have received at your new GP Practice.
