About: Virtual Wards / Frailty Virtual Ward

(as a staff member posting for a patient/service user),

I felt very safe after being discharged and being handed over to the nurses at the virtual ward service, when I came out of hospital I got poorly again, it made me feel so assured knowing I had the team at the end of the phone, they got me referred to GP, changed my medications and got me back on the straight and narrow, I don’t know what I would have done with out them. I knew if I needed to I could call them too.
