Patient's Mum/Carer was very appreciative and thankful for the care he received with our team, especially the medical care which resulted in him getting discharged from hospital quickly. I have let her know that the praise for medical care should go to Dr Budha as she has been leading on his care – completing senior review on admission, coming up with a treatment plan, reviewing him to see if the treatment plan was appropriate and collaborative and she led the discharge CPA as well. Thank you!

Carer and Patient were also very appreciative of the care and support provided to him on the ward, keeping him engaged and for the clear plan written down for him on discharge. Thank you for a fantastic job, ward team and Discharge Facilitator for the written discharge plan.
