"Support put in place"

About: Community Assessment Support Team (CAST)

(as a relative),

Stacey has put in a care package in place quickly for my Uncle, which means he will now get support during rehabilitation in the comforts of his home.  I spoke with Stacey for the first time yesterday and less than 24 hours we've been able to map out next steps together and put a plan in place that puts my Uncle at the heart of the care he needs to regain his quality of life back.

 Stacey signposted some suggestions for me to action and follow, which I did, and I appreciate the guidance and support currently being provided.  Stacey is easy to contact and speak with - thank you.

I am delighted that your uncle had a care package put in place quickly meaning he can stay at home during his rehabilitation and receive support. It sounds as though Stacey is providing person centred care for your Uncle, which is fantastic!

I am sure Stacey will be delighted with your feedback and knowing that she has helped to make such a difference to you and your uncle.
