"Community Care Assessment and care home placement"

My mother was admitted to Coathill Hospital last year. It was clear from outset that she would not be able to live independently at home. After a very complicated but necessary process my sisters and I were awarded Guardianship in January. 

We would like to take the opportunity to thank Angie ( Hospital SW) for her CCA contribution and Michelle( MHO) for speedy response to MHO assessment request to inform Guardianship application. 

Our mother has now been living in a care home placement for several weeks and her review has noted her to have settled and put on weight. More importantly for us is that we know she is safe. 

Angie and Michelle both went above and beyond in keeping us informed. We all work and facilitating face to face meetings was difficult so we were really grateful for their flexibility of response and commitment to keeping us advised of process. Thank you!!!
