"Transition from Children to Adult Services."

About: Downe Community Services / Childrens Community Nursing

(as a parent/guardian),

Our daughter has been transitioning from Community Children's Nursing Services to Adult services for two years and in April she will become an adult.  After numerous professionals involvement and assessments we still have no plan in place. Our daughter takes Laryngospasms ( a condition where her airway is restricted due to her vocal chords sticking together). Throughout Primary and Secondary Education she received medical assistance via 2 (different annually) classroom assistants trained by the  children's nursing team. These assistants could assist our daughter with her oxygen and rescue medication if required. Also the children's nurses trained Health Care Assistants for our daughter to engage socially with other young people via Youth Club and day trips with IncredAbles! 

So for our daughter to access Further Education, a basic human right, she requires a trained medical assistant to accompany her and she also needs to engage in social activities for her overall well being. But 22 months later, we are no further forward despite tireless efforts from our Transition Coordinator Officer and CCN team along with numerous professionals including an Advocate from Disability Action and an MlA. 

The system to date  has failed my daughter.  The insurmountable, unnecessary stress as her next birthday is hurtling towards us. This kind, caring , capable young girl who has weathered many storms and climbed many mountains and deserves better. She is worried she will be lonely at home every day with no purpose .

We feel there is systemic lack of accountability within Adult Community Services. If there is any way you could shed light on these problems it would much appreciated.

I am sorry to hear of the difficulties regarding your daughter's care needs and apologise to you for each element that should have been better looked after.

I have shared your story to service leadership for consideration and effective response and I will post an update here.
