"Medication mix-up"

About: Forth Valley Royal Hospital / Accident & emergency Forth Valley Royal Hospital / Acute assessment unit

(as the patient),

I was blue lighted into the hospital in December just after 10:00 am.  I was in A&E and then was sent to AAU and  AAU3.  I was in the hospital for 3 to  4 days before they decided to send me home.  It was 4:20 before I got home. They had known for some days that I was going home so I think they could have sent me home at a decent time of day.

I was home for 5 days and the hospital pharmacy contacted to tell me that they think they have made a mix up in my meds.  I checked my pills they were all okay proper strength and amount, but my inhaler was double the amount I'm on.  Fortunately I hadn't opened the one they gave me so I will return it to my local pharmacy and get them to exchange it.

It feels completely ridiculous getting meds from the pharmacy.  It is bad enough being given the wrong pills in the hospital.  They just laughed about the wrong strength being given and said it would have only made me light headed - as if I wasn't dizzy enough with the pills I'm on.

I look forward to you taking this in hand and sorting it out before someone else is given the wrong meds.

This is not our normal standard of patient care and we have conducted a thorough investigation as to how this came about and, more importantly, how this made you feel when you were receiving the information about it. We will ensure that our whole team learn from this and have put processes in place to stop this from happening again. "
