"Incorrect discharge letter"

About: Lincoln County Hospital / Paediatrics

(as a parent/guardian),

I’ve just received a discharge letter for my 3 yr old son. We came in purely for a blood transfusion, organised by our usual hospital. The discharge letter states we were admitted under emergency circumstances and presented with dehydration. It also states he has a glioblastoma when in fact he has a very different type of tumour. This misinformation is dangerous in his health records.

Thank you for your feedback - we are sorry to hear that your son’s discharge letter contained inaccuracies and agree that this does not meet our usual standards. We work hard to ensure that all aspects of our clinical documentation contains factually accurate information that reflects the treatment that a child or young person received whilst in our care.
