"My gran passed away as a direct result of toxicity"

About: University Hospital Ayr

(as a relative),

During my grans admission she was prescribed a drug used to treat a condition she didn’t have. The drug is widely known to cause renal toxicity and my gran had chronic kidney disease. She was also prescribed almost four times the dose which she should have been. The correct diagnostic tests were not carried out and none of the reviewing doctors or administering nurses noticed the dose error. She received 7 doses of this drug before the error was noted.

She deteriorated significantly during this time and became quite agitated. Her agitation was poorly managed by the ward staff causing her further harm. They refused to let any family members visit during this time due to ‘covid restrictions’. The communication from the whole team was poor and I believe that the nurses and doctors responsible for her care did not communicate with each other either.

Within a few days of the drug error being noted, my gran passed away.

After a full investigation by Ayrshire and Arran I am not aware of anything that has been changed to prevent this happening again to somebody else and nobody is taking responsibility as far as I can see.

My gran would have been much safer staying at home. She was sent to Ayr hospital by her GP for minor issue. If she’d have avoided Ayr hospital then she might still be alive.

Hospitals first and foremost should be a place of safety and Ayr hospital completely failed to provide my gran with the most basic care and instead caused her great harm.

Thank you for your post. Please allow me to express my sincere condolences of the death of your loved one. I am so sorry that you have remaining concerns about a potential adverse event causing harm to your gran whilst in the care of university hospital, Ayr. This is certainly not the experience we wish for any of our patients and their loved ones.
