My mothers care at this hospital has been appalling. Twice I was told incorrect information about her care. Once I was told she had a gastrostomy tube inserted in order to feed her, on the second occasion I was told she was in ITU following her operation and that she was in pain. The nurses on the ward clearly do not know what is happening with the patients in their care. Despite my telling the staff that my mother has a short term memory issue and that she requires information in written form and a family member requires to be informed of discharge arrangements. No information has been given to her or her family and due to her memory difficulties she has no idea of future follow ups. Her medications were altered according to the pharmacist but no action was taken and no information provided. She is not safe to be sent home because when she eats she chokes, coughs and splutters, the ward staff have not put any risk management plans into place to prevent this such as care input/ making sure someone is home with her.
