I think that when you are being discharged from a Hospital Ward it would be usefull to receive a discharge summary check list ie a discharge letter, how much medication you have taken up to your discharge what you have left to take, how to dispose your sharpes bin, the offer of a sick note if needed, information regarding time scale for your follow up clinic appointment ,information about dos and donts and what to expect following surgery and a phone number for any problems that you may encounter when you are home.

Thank you for your feedback. I am sorry you are unhappy about the lack of information provided to you upon your discharge from The James Cook University Hospital.

South Tees NHS Foundation Trust expect every patient to receive the information on discharge as you have described and the ward manager apologises that you were sent home without this.

At the moment, staff on the Gynaecology Ward do not routinely give the patient a copy of the discharge letter but they are currently addressing this issue.
