About: Telford referral and quality service (TRAQS)

(as the patient),

After a referral from Dr Parish of Dawley Medical Practice, I very soon had my first appointment for physiotherapy in Madeley. Ben the physio did his best for me but referred me back to Dr Parish after four sessions. I am quite happy with the system so far but have now gone on a different referral through Cannock Chase Hospital and now have another TRAQs form to be completed eventually.

Many thanks for taking the time to post your feedback. Im sorry that the physio has not helped. As it was then felt by your GP to refer to a hospital it would need another referral letter to be sent to TRAQS. I hope that Cannock Chase manage to sort out your helath problems.
