I changed my phone number and address but still received calls and post to my old number and address.

Dear ‘Pattern723’,

At TRAC we are able to contact you using the information given to us from your GP with your referral. It sounds as though the information held by your GP may not have been updated prior to your referral, you may wish to check this information for future correspondence. If you are due to be seen in a hospital department or clinic you may also wish to check your details with them. I would be happy to check this for you if you are able to provide me with some more details about your referral.
