"I had to pay postage for my warfarin record book"

About: East Surrey Hospital / Clinical haematology

(as the patient),

My warfarin record book was posted without payment being made. I had to go to the post office and collect 2 weeks after the test and pay 1. 5 pounds. After phone calls and postage this has cost me nearly 3 pound and a lot of hassle.
