Far too much time spent by nurses filling in forms and not enough time spent on nursing care or with patients.This is of no use if the information filled in on the forms is incorrect. My mother was classified as independent when she had just come on the ward from the high dependency unit and was definitely not independent. She got very distressed when no one would help her to get washed and dressed. In fact my sister and I did it ourselves when we arrived on the ward. Communication was poor regarding my mothers condition and progress. However the occupational staff were very good.

Nurses are required to divide their time between direct patient care and completing forms keep patient notes up to date and ensure the best and most appropriate treatment is given. However, we are very concerned that some of your mother’s details appear to have been entered incorrectly. We are truly sorry if your mother felt distressed while she was in our care as a result.

Unfortunately it is difficult to comment further on your specific case without your mother’s details and reviewing her medical notes. We would welcome the opportunity to look into the issues you have raised and to find out which nursing team cared for your mother.
