(as the patient),

I had a phone call last night from one of your members of staff, she did apologise for the inconvenience caused, and gave me another time and date to attend hospital.

Thank-you for the incentive to rectify the situation but really I shouldn't of had to prompt you.

I hope this matter of telling people to attend non-existent appointments will be considered with high regards, as I do not wish anybody to receive the same inconvenience as me.

I'm sure this was just a slight over-sight but should be taken quite seriously.

Thank you for eventually sorting out this matter.
