"Referral to Multidisiplinary Team clinic East Leeds health Centre for physio to attend patient with parkinsons disease."

My mother was referred by the Elderley Services consultant after her consultation in October. It is now 27th December and no appointment has been made. I contacted the consultants office before Christmas to chase this up, my mother received a telephone message to say she was on a waiting list. In the mean time my mothers condition has worsened and she has fallen twice and has great difficulty in walking and standing. I have tried to contact the East Leeds Health Centre today. They tell me she is not on their list? I have contacted the consultant's office. They have given me a number to ring which is the MSK Admin no and all I could do was to leave a message on the answering machine.
