About: Telford referral and quality service (TRAQS)

(as the patient),

I have been to your place before and have had very good service. I will be there on the 30th at 4. 50pm when I will be attending the dermatology outpatient department.

TRAQS are a new service providing help and support for the patients registered with GPs within the NHS Telford and Wrekin area when they require a referral to a hospital or community clinic for further treatment.

We are happy that you have received a very good service from the team you have been booked with and hope that on this occasion you are as happy with the service as on previous occasions
