About: East Surrey Hospital / General surgery

(as the patient),

I was admitted in to East Surrey Hospital in March to undergo the above. I was operated on the following day.

I have since suffered in pain due to nerve damage that was caused by this operation. I wish for a full explaination as to why this was caused to be given to me as soon as possible.

I have had on going treatment since this operation and have been advised that this damage should not have been caused by a routine operation as this.

I look forward to hearing from you as a matter of importance.
