Dear Sir/Madam

Thank you so much for taking the time to provide us with your feedback.

It is always a pleasure to hear of the positive experience that a patient has had with our service and that this was continued throughout your contact with other NHS staff.

I am very glad to hear that you are well on the road to recovery. If you would like me to pass on your comments onto the NHS Direct staff involved in your call, please forward the following details to me via our confidential 'Contact Us' facility at www.nhsdirect.nhs.uk. Your name/address including postcode, date of birth and the telephone number you called our service on; this will enable us to locate the staff member you spoke to.

Thank again for your feedback.

Yours sincerely
