If you have decided to relate your recent care experience, 'take a moment', an adequate allotment of time in which to do the subject justice? Just write (or draw). Seems to suggest your invitation has a frivolous aspect to it. If my opinion is 'to make a difference', then it surely requires me to spare more than a moment to form an opinion and express it on paper.
I have too much respect for the NHS, the doctors and nursing staff involved in treating me to fob them off with a casual response.
Unless--a usual response is all that is required. The amount of space left for a written opinion suggests it might--therefore, before committing myself to paper, I rang the number below to ask for directions as to what is wanted and spoke to a clerk who directed me as follows
"Patient Opinion's freepost feedback leaflets"
